Saideman Practice terms & conditions

(References below to “the Practice” are references to Dr Saideman’s Practice).
Registration: All patients wishing to register with the Practice will need to provide a valid form of photographic identification (eg. passport/driving license) showing proof of address within the last three months. For children (ie, patients under the age of 16), this will need to be delivered by their parent or guardian.Patients are requested to print out the Registration Form from the website for completion and should bring the completed Registration Form to the first appointment (alternatively the Registration Form can be completed at the surgery prior to consultation). A Registration Form must be completed and signed (together with any other paperwork) at the first consultation.
Fees and charges: All fees and charges are payable to Dr Saideman at the time of consultation by cash or cheque (with a cheque guarantee card). Please see the website or contact the Practice for the full range of fees and charges. Please note that prices for all fees and charges are subject to change. Dr Saideman shall be permitted to charge an additional amount (above the stated fees) in the event that the time spent for a consultation exceeds the usual designated time for such consultation (at Dr Saideman’s discretion).
Pathology charges for the analysis of any tests, swabs or other samples and any charges for scans/procedures are not included in the cost of consultation. Prices for such items are available on request at the surgery – the prices vary enormously depending on the nature and scope of the test and patients are advised to ask at the time of consultation as to the expenses to be incurred. All such expenses are payable to the Practice at the time of consultation. Charges for administrative procedures such as completion of insurance claim forms, medical reports, passport application forms are in addition to any consultation fees (please ask for details of such charges). Charges for referral letters may be subject to an additional charge.
If any fees or charges remain unpaid after the date for payment interest will be charged at a rate of 5% p.a., such interest to accrue from the due date for payment until payment on full. All sums are stated exclusive of VAT. As at the date of these terms and conditions VAT is not payable on services to patients. If the services subsequently become subject to VAT, this will be charged in addition at the applicable rate.
Consultations: Please take note of the consulting times and office hours as on the Practice website. The Practice will agree with patients the time and date of appointment. Failure to attend an appointment by a patient may result in reallocation of the appointment slot to another patient. If a patient is unable to attend an appointment, the Practice requires at least 24 hours’ notice (or, in the case of a same-day made appointments, at least 2 hours’ notice) in order to reallocate your appointment to another patient. Failure to give the required notice may result in your being charged a cancellation fee.
When booking a consultation, please be aware that multiple or complex problems may require a special consultation (beyond the routine length of time) and will be charged accordingly.
Patients are asked to arrive for their appointment on time so that any necessary paperwork can be completed. Late arrival may result in reduced treatment/consultation time or forfeiting of the appointment. The Practice will endeavour to ensure that appointments runs to time, however, for reasons beyond the Practice’s control, the Practice may need to delay, cancel or postpone an appointment at short notice.
Telephone advice from Dr Saideman is offered only to registered patients of the Practice. For new problems registered patients will be routinely advised to make an appointment to see Dr Saideman. In certain circumstances Dr Saideman may deem it appropriate to offer telephone advice for new episodes. Any consultations or advice over the telephone will be chargeable depending on the amount of time spent and the actions taken in connection with the consultation.
Home visits may be available in certain circumstances in the sole discretion of Dr Saideman. The consultation fees for such home visits are available on request or on the website (and are also subject to change)
Liability:  Dr Saideman (and the Practice) will not be liable in contract, tort (including negligence) or otherwise for any economic loss (including, without limitation, loss of profit), or for any other special, indirect or consequential loss or damage arising out of, or in connection with, the provision of goods or services. Nothing in these terms and conditions excludes or limits liability for death or personal injury directly caused by the Practice’s negligence.
It is the responsibility of the patient at all times to ensure that all relevant medical details are provided prior to any treatment. Dr Saideman will not be liable for any damage that occurs as a result of the patient’s failure to disclose such details.
The patient acknowledges that Dr Saideman (and the Practice) gives no warranties or representations to the patient (whether express or implied) in respect of any third party services provided to Dr Saideman and/or the Practice relating to the patient (eg, pathology services or other third party services). Dr Saideman (and the Practice) accepts no responsibility for any error or defect in any test results, scans or other third party results, scans or reports.
Subject to the provisions of these terms and conditions, Dr Saideman’s (and the Practice’s) total liability in contract, tort (including negligence), misrepresentation or otherwise arising out of the performance and provision of services will be limited to the amount payable by the patient for such particular service.
Privacy:  Any personal information you supply to the practice will be treated in accordance with the Data Protection Act 1998.
Patients will be asked (when they register with the Practice) to sign a Patient Consent Form in connection with the processing and/or disclosure of personal information. Patients are to ensure that all personal information they provide to the Practice is accurate and updated and shall inform the Practice in writing as soon as possible of any changes to such personal information.
These terms and conditions shall by governed by and construed in accordance with English law.
